This Privacy Notice applies to information we collect when you contact us via telephone or when you visit MDhome's main website (www.mdhomecare.com) or other websites or mobile applications that we own, control, or link to this Privacy Notice (collectively, the "MDhome Technology").
This Privacy Notice describes how medical and other information about you is collected through your use of the MDhome Technology, how such information may be used and disclosed, and how you may access it. Please review this Privacy Notice and the Terms of Use carefully before using any part of the MDhome Technology. By using MDhome Technology, you consent to the terms outlined in this Privacy Notice, our Terms of Use, and the HIPAA Notice. If you disagree, please refrain from using the MDhome Technology.
1. Collection of Information
A. Information You Provide Directly
When you register as a user, book services (e.g., a house call or telehealth visit), subscribe to alerts, or contact us, we may collect:
I. Full name, email address, phone number, and mailing address;
II. Username and password;
III. Payment details, including credit card information;
IV. Health information including symptoms, diagnoses, past treatments, and other health-related data;
V. Reviews of providers;
VI. Any other information you choose to provide.
We may combine this with information already on file.
B. Information Collected Automatically
We may collect details such as your IP address, browser type, operating system, device identifiers, location data (e.g., GPS, WiFi), pages visited, time and date of access, and interactions within MDhome Technology using cookies, pixel tags, and similar technologies. Use of MDhome Technology implies consent to these practices.
C. Information from Third-Party Services
We may receive information from third-party websites or applications you use to access MDhome Technology.
D. Information from Healthcare Providers or Other Sources
In connection with healthcare services (e.g., booking a visit), we may obtain medical records from other healthcare providers, including test results, clinical notes, family history, and relevant correspondence. We may also combine this with third-party data.
2. Use of Information
We may use your information to:
Deliver and enhance MDhome Technology and services
Contact you for operational or promotional purposes
Fulfill service requests
Send updates or wellness information
Improve user experience and functionality
Conduct authorized research
Prevent misuse, fraud, or violations of our Terms of Use
As disclosed at the time of collection or as permitted with your consent
3. Protected Health Information (PHI), HIPAA, and Communications
Some information collected or generated may be PHI under HIPAA. While MDhome is not a "covered entity," our partnered Providers may be, and thus subject to HIPAA. MDhome may serve as a "business associate" to such Providers.
As such, we may send PHI via text, email, or app-based communication, including appointment reminders or health updates. If you prefer not to receive such communications, please email privacy@mdhomecare.com. Requests to access or correct PHI can also be submitted via this email, though retention requirements may apply.
4. Sharing of Information
A. Third-Party Vendors
We may share your information with vendors providing billing, tech, medical, customer service, analytics, marketing, or hosting services. These parties may only use your data to provide services to us.
B. Affiliates
Your data may be shared with MDhome affiliates.
C. Legal Requirements
We may share your data as required by law, court order, or to protect the rights and safety of MDhome, our users, or others.
D. Business Transfers
We may transfer your data in the event of a business sale, merger, or reorganization, in compliance with HIPAA
E. PHI
PHI may be transferred per HIPAA and Provider HIPAA Notices.
F. With Consent
We may share your data as instructed or approved by you.
5. Security
We implement safeguards to protect your data, including PHI, from loss, theft, misuse, and unauthorized access or disclosure. However, no internet transmission is fully secure.
6. Your Choices
You can refuse or disable cookies in browser settings. Some features may not function without them. We do not share PHI with unaffiliated third parties for direct marketing.
7. Third-Party Links
MDhome Technology may link to third-party content. Their privacy practices are not governed by this Notice.
8. Limiting Data Collection and Do Not Track
A. Opt-Out
To opt out of interest-based ads, visit Digital Advertising Alliance or Network Advertising Initiative sites. You must opt out on each device or browser separately.
B. Do-Not-Track Signals
We do not respond to Do-Not-Track signals.
9. International Users
Data is stored in the U.S. and governed by U.S. law. By using MDhome Technology, you consent to data transfer to the U.S.
10. Children
We do not knowingly permit users under 18 to create accounts unless legally emancipated.
11. Updates to This Notice
We may revise this Privacy Notice. Updated versions will include a new effective date and be posted to our site. Please review periodically.
12. Copyright and Trademarks
MDhome owns all rights to its technology and content. No license is granted for use of MDhome trademarks or service marks.
13. Legal Disclosure
