Provider Privacy Policy Notice for MDhome

Effective Date: June 2nd, 2025

When you book a visit through MDhome, the medical services—including in-home and virtual healthcare—are provided not by MDhome, but by independently practicing physicians and licensed providers in your state (the “Providers,” “we,” “our,” or “us”). MDhome is a technology platform that connects users like you to Providers; it is not itself a medical provider. This Privacy Notice describes how medical information about you may be used and disclosed by the Providers, and how you can access this information. Please review it carefully.

You have the right to:

  • Get a copy of your paper or electronic medical record

  • Correct your paper or electronic medical record

  • Request confidential communication

  • Ask us to limit the information we share

  • Get a list of those with whom we’ve shared your information

  • Choose someone to act for you



File a complaint if you feel your rights are violated.

When it comes to your health information, you have certain rights:

  • Get an electronic or paper copy of your medical record

    Ask us how to do this. We will provide your visit summary and can share your record with your primary care provider.


  • Ask us to correct your medical record

    If you believe health information about you is incorrect or incomplete, request a correction. We may say “no,” but will explain why in writing within 30 days.


  • Request confidential communications

    Ask us to contact you in a specific way (e.g., home or office phone) or to send mail to a different address. We will say “yes” to all reasonable requests.


  • Ask us to limit what we use or share

    You may request that we not use or share certain information for treatment, payment, or our operations. We are not required to agree and may say “no” if it would affect your care.


  • Get a list of those with whom we’ve shared your information

    You can request an accounting of disclosures for six years prior to your request. This includes who we shared it with and why, excluding disclosures for treatment, payment, and health care operations.


  • Get a copy of this privacy notice

    You can ask for a paper copy at any time, even if you agreed to receive it electronically.


  • Choose someone to act for you

    If you have given someone medical power of attorney or if someone is your legal guardian, they may exercise your rights and make choices on your behalf. We will confirm their authority before acting.


  • File a complaint if you feel your rights are violated

You may tell us your choices about what we share. In these situations, you have the right and choice to tell us to:

  • Share information with your family, close friends, or others involved in your care

  • Share information during disaster relief

  • Include your information in a provider or facility directory



Typical Uses and Disclosures of Your Health Information

To treat you:

We may use your information and share it with other professionals who are treating you.
Example: Sharing visit notes with your primary care provider.

To run our organization:

We can use and share your health information to run our practice, improve your care, and contact you when necessary.
Example: Using health data to manage and coordinate services.

To help with public health and safety issues:


We may share your health information to:

  • Share information with your family, close friends, or others involved in your care

  • Share information during disaster relief

  • Include your information in a provider or facility directory



To comply with the law:

We will share information if required by federal or Washington State law, including with the U.S. Department of Health and Human Services.
With medical examiners or funeral directors:

We may share your information in the event of your death.

To address workers’ compensation, law enforcement, or government requests:

We may share your information:

  • For workers’ compensation claims

  • With oversight agencies for audits or inspections

  • With law enforcement as required or permitted by law

  • For special government functions such as military and national security


In response to lawsuits and legal actions:
We may share your health information in response to a court order, subpoena, or administrative request.

Our Responsibilities

  • We are required by law to maintain the privacy and security of your protected health information.

  • We will notify you promptly if a breach occurs that may have compromised your information.

  • We must follow the practices described in this Notice and make a copy available to you.

  • We will not use or share your information other than as described here unless you give us written permission. You may revoke that permission at any time in writing.
